Passport Services

The Office of International Education and Development at Florida Agricultural and Mechanical University is a passport acceptance facility. We accept passports for FAMU students and employees, as well as members of the community.

OIED Passport Services

The Florida A&M University Office of International Education and Development office is officially designated as a Passport Acceptance Facility by the US Department of State. We process passports for students and employees, as well as members of the community.

Blue American passport and other Personal Documents

Passport Books And Passport Cards

  • A passport card allows you to travel via land or sea to Canada, Mexico, the Caribbean, or Bermuda.
  • International air travel is not permitted with the passport card.
  • As a Passport Acceptance Facility, the Office of International Education and Development is authorized to receive your completed application for a US passport book/card and submit the application along with all necessary documentation to the US Passport Services. The US Passport Services will then decide eligibility for a US Passport.
  • It is important to note that only the US Department of State can make the final decision to issue a passport to an individual.

  • Note: Due to COVID-19 the US Department of State is operating with a nationwide back log of applications. Because of this, passport processing times are longer than pre-pandemic processing times. For more information, please visit

Requirements to Apply for a Passport Book or Passport Cards

Please Read Thoroughly!

All applicants must have:

1. Complete Form DS-11 (application for a US passport). Available online or in the Office of International Education and Development.

2. Submit evidence of US citizenship (examples include: previous passport, original/certified copy of your birth certificate, original naturalization certificate). Photocopies are not acceptable. The US Department of State requires long-form birth certificates, which requires your parental information to be visible.

3. Photo Identification (examples: valid US driver’s license or State ID). Non-Florida residents may be required to provide a second form of ID for verification. Ensure you have additional photo verification.

4. Passport Photo. We do not offer passport photo service onsite. You will need to arrive with your passport photo taken prior to your application. 

5. Applicable fees. When processing your passport, two separate fees are collected. Only check or money order are accepted to pay for the fees; starter checks are not acceptable.


Applicants age 16 or 17, in addition to information listed in numbers 1-5:

6a. If an applicant is within this age group, do note there should be parental awareness when applying for the passport book/card. Parental awareness can be in the following forms: parents can write a check for the fee to the US Department of State or the applicant can have at least one parent/guardian with them at the time of application processing.


Applicants under age 16, in addition to information listed in numbers 1-5:

6b. Both parents/legal guardians and the child must be present to execute a passport for children within this age group. If one parent is not available to attend the passport appointment, form DS-3053 must be completed in place of the parent.

7.Drivers licenses of both parents/guardians.

8. Child’s evidence of US citizenship (examples include: previous passport, original/certified copy of birth certificate, original naturalization certificate)


ADDITIONAL REQUIREMENTS:

Proof of Name Change: If your name has changed for any reason (e.g. marriage, adoption, etc.), you will need to bring in original documentation (marriage certificate, adoption papers, etc.).

Please note: All fees must be paid by check or money order. Fees for the U.S. Department of State and fees for Florida A&M University must be on separate money orders or checks.

Note: Application processing times do not include mailing times, which will vary. Florida A&M University Passport Acceptance Facility does not guarantee approval of a passport or processing times for a passport. Approval is determined by US Passport Services only.  For additional information, view the passport information from the US Department of State. The fees below are separate and may not be combined with the fees paid to Florida A&M University.  Make the fee payable to "U.S. Department of State" via check or money order only.

The fees below are separate and may not be combined with fees paid to US Department of State. Make the fee payable to "Florida A&M University" via check or money order only.

Renew your passport by mail or online:

One qualifies for renewal by mail if their most recent US passport meets the criteria below:

• Current passport is not damaged and is available to be submitted with the application

• Current passport was issued at the age of 16 or older

• Current passport was issued within the last 15 years

• Current passport was issued in current name or a legal document can be provided to show proof of a name change.

If you do not meet the criteria as outlined above, you must complete form DS-11 and apply for a passport in person.


FAMU’s passport acceptance facility does not accept applications for renewals. Renewal applications must be mailed off by the applicant. A passport agent can review the passport application and inform the applicant if their information is correct. An applicant can pick up the renewal application form DS-82 at the FAMU passport acceptance facility.

  • Prior to arriving for the scheduled appointment time, it is important to thoroughly read the information sent in the appointment confirmation email.

  • Appointments must be scheduled at least 24 hours in advance.

  • Families can be scheduled as one appointment. Please email us in advance if you intend to submit more than two applications during your allotted appointment time.
